I hereby authorize
the directors of the Farmington High School Basketball Camp/Clinic to act for
me according to their best judgment in any emergency requiring medical
attention, and herby waive and release Farmington Basketball Camp/Clinic from
any and all liability for any injuries or illness incurred while at the Farmington
High Basketball Camp/Clinic. I will be
responsible for any medical and/or other charges in connection with my son’s or
daughter’s attendance. I know of no
mental or physical problems which may affect my child’s ability to safely
participate in this program. All money for this clinic goes directly to paying
coaches for their time and to the Farmington Basketball budget for equipment
for players.
